What is going on with my eight year old?

I have an eight year old daughter and ever since shes been in school shes been afraid to use her voice more, like not being shy with other kids in a clroom. Shes quiet in cl but when she goes out for recess shes loud with the children. She had skating today and she handed in her form to the teacher last week but today when she was to go skating her teacher told her that she had to stay behind because she didn't get her form, instead of my daughter telling her "I did hand it in" she didn't say anything and stayed behind. Also, I notice that on a regular basis I have to tell her to brush her teeth first thing in the morning, take a shower, and other such daily routines that she should be used to by now, I been telling her these things since was was a toddler and she still doesnt' do these things on her own. I don't know what the reason for all this is. What could be going on with my child and how do I help the situation? Thanks for any help!
